ORDER

of information requests composition and submission

1. Request for information may be submitted by individuals and legal entities, associations of citizens without “legal entity”- status to the Defence Intelligence of Ukraine orally or in written form.

2. Orally, request for information may be submitted either during personal reception of citizens (hereafter referred to as – during personal reception) by the Minister of Defence of Ukraine, Deputy Defence Ministers of Ukraine and the Chief of the Defence Intelligence of Ukraine, or by the phone.

3. Such a request is made in a free form, but it should necessarily contain:
name, surname of the individual or name of the legal entity making a request, postal address, e-mail, phone number or fax (if any);
general description of information, type, name, requisites or content of the requested document (if the person, submitting a request, knows it);
way of information acquisition.

4. During the reception, the information request is composed by the person, submitting it; the request, lodged over the phone is made by the responsible employee of the Public Affairs Service of the DIU Administrative Department.

5. The written request for information is sent by post, e-mail, telefax or during a personal reception, according to the form established in the Defence Ministry of Ukraine.
